1. Required to sign a Baltimore County Authorization for Release of Medical Information form or any other form of medical records release, or
2. Ordered to undergo a “Fitness for Duty Examination”, “Independent Medical Evaluation”, “Workability Examination” or
3. Required to be examined by Dr. Oroszlan
